Senior Clinic

LCHC’s Senior Clinic is a specialized wing in our Main Clinic building offering a dedicated space for patients aged 60+ to receive their medical care. Here’s what you can expect at the Senior Clinic:

  • Separate entrance and waiting area

  • Seniors-only exam rooms

  • Geriatric specialist

  • Transportation to and from appointments

  • Accessible parking lot and building entrance

  • Services offered in English or Spanish

Making an Appointment

To make an appointment at our Senior Clinic, call us at 872-588-3000. Our Senior Clinic is located in the east wing of our 3860 W. Ogden Avenue building. Enter through the east parking lot, as indicated on the map below.

Healthcare at Home

Some days, getting to the clinic may be challenging. We offer the following services to make sure you get the care you need, when you need it.

Telehealth

You can make an appointment to speak with your Provider on the phone or through a video call. Simply call your care team and request a telehealth appointment.

Medicare Wellness Exams

Patients with Medicare are required to complete a yearly wellness exam in order to maintain their health plan. LCHC offers these annual wellness visits in the comfort of your own home!
