Adult Health

Our Family Practice and Internal Medicine specialists are dedicated to providing our patients with comprehensive healthcare options. Primary care services include: physical exams, vaccinations, anxiety and depression screenings, chronic disease management (e.g. asthma, diabetes, hypertension), and referrals for specialty care. Adult healthcare is offered at all of our clinic locations.

Behavioral Health

Behavioral Health

Behavioral Health psychology services are integrated into our Primary Care. BH providers address areas such as mental health concerns, coping with chronic illness and life problems, and behavioral change assistance. BH providers are licensed psychologists and pre- and post-doctoral trainees.

Services include:

  • BH consultations for mental health
  • Behavioral Medicine screenings

Children’s Health

Children's Health

LCHC offers a wide range of comprehensive primary care, dental, optometry, and support services to our pediatric patients and their families, including:

  • Well-Child visits
  • Immunizations
  • School and sports physicals
  • Sick visits

Dental

Dental

Our high-quality dental services for adults and children are offered at each of our two dental clinic locations, at the Health & Fitness Center and Homan Square. You must have a primary care provider at LCHC in order to receive dental care. Call our dental line at 872.588.3220 to schedule an appointment!

Services include:

  • Routine exams and cleanings
  • Fluoride treatments, sealants, and x-rays
  • Fillings and crowns
  • Extractions, root canals, fixed bridges, dentures, scaling and root planing
HIV Care

HIV Care

LCHC offers a multidisciplinary approach to caring for patients living with HIV, as well as those at risk for this disease or affected by it in some other way. All services are available in English and Spanish, and include:

  • HIV Medical and Preventive Care
  • Case Management and Support Groups
  • HIV Testing and Counseling
  • Community Outreach and Education
  • Peer Support

Immediate Care

Immediate Care

Our Immediate Care Clinic provides urgent medical attention for patients with an illness, minor injury, or infection. Patients of LCHC can be seen at Immediate Care on a walk-in basis. Click here to learn more about what we treat at ICC. Please note: this is not an emergency department. If you are experiencing a medical emergency (life-threatening symptoms) please call 911 or go immediately to your nearest Emergency Department.

 

Mobile Health

LCHC’s Mobile Health Team provides healthcare services within several partner homeless shelters across Chicago. The team makes regular visits to these shelters and is equipped to provide medical visits, limited prescriptions, behavioral health services, and more at no cost to shelter residents.

Vision Care

Optometry

Our eye clinic provides comprehensive eye exams for current medical patients. Our doctors are experienced with examining and treating patients with Glaucoma, Cataracts, Diabetic Retinopathy and the ocular effects of other medical conditions.

We carry a large selection of frames at low cost and participate in the Illinois Medicaid eyeglass program.

Pharmacy

Our Pharmacy collaborates with our medical staff and care teams to provide quality services for our patients. We accept many Medicaid and Medicare insurance plans. Please give us a call or stop by with your insurance card and we can check if it is a plan we accept.

Specialty Care

Specialty Care

LCHC provides limited onsite specialty services (such as x-ray) which can be requested by your PCP or an Immediate Care provider. If you need a referral for specialty care such as Cardiology, Rheumatology, Orthopedics, and more, talk to your PCP during your appointment or contact your care team's referral coordinator using the number found here. They will help you find a specialist within the Lawndale network or covered by your insurance plan.

Substance Use Treatment

LCHC offers specialized treatment for patients with a substance use disorder (SUD). Many of our Providers have received certification to offer Medication Assisted Treatment (MAT), which is available at all of our clinic sites. MAT combines the use of medication and behavioral health therapies to treat SUD, sustain recovery, and prevent overdose. LCHC's Recovery Community meets in The Gallery and offers group therapy and activities for patients on their recovery journey. Walk-ins are welcome! Click here for more information.

 
Women’s and Prenatal Care

Women's & Prenatal

Our excellent Women's Health providers offer women's well-care services, such as gynecological exams and well-woman visits. LCHC offers free pregnancy tests at all of our sites on a walk-in basis or by appointment. Prenatal care is offered in two models: Traditional pregnancy care or Centering pregnancy groups.
