How do I get a refill?
- To order electronically, please use the tool below. The prescription or “Rx” number is located on the medication bottle above the patient’s name. If you have multiple medications, please click “Add Rx number” to add additional entries.
- To order by phone: 872-588-3250 and follow the prompts – the prescription or “Rx” number, is located on the medication bottle above the patient’s name. When instructed, please enter the 6-digit number of the medication (omit any letters before or after). If you have multiple medications, please enter 1 Rx number, verify that it is correct, and follow the prompts to add another Rx number. Enter the Rx number for each medication needed.
- If you have lost your prescription bottle or Rx number, you may press “0” to speak with pharmacy staff, and place your order by telling them your date of birth and the name of the medication.
- You may also order in person at the pharmacy, either with the prescription bottle, Rx number, or patient’s date of birth.
- If the bottom of your prescription label says “No Refills” or “Refills until (date)” and the date has passed, you must contact your doctor or his/her nurse to obtain a new prescription before any medication can be dispensed.
How much should I expect to pay for my medicine?
- Prices vary depending on the medication and quantity prescribed. In addition, prices may fluctuate based on changes in the prices LCHC pays to purchase the medications. Please check with pharmacy staff for pricing of your specific prescriptions.
Who do I call if I have a problem with my medication?
- If your medication appears different than it did last time, you think you may be experiencing a side effect or unexpected reaction, or you have any other question about your medications, call pharmacy staff: 872-588-3250 – press 0 to exit automated refill system and speak with pharmacy staff. If you have swelling of the face or mouth, or are having difficulty breathing- call 911 or go to the nearest hospital’s emergency department immediately.
- We accept many Medicaid and Medicare insurance plans. Please give us a call or stop by with your insurance card and we can check if it is a plan we accept.
