Lawndale Christian Health Center Wins $50,000 Grant for Continuous Patient Care Improvements
Lawndale Christian Health Center (LCHC) has recently been awarded a $50,000 grant from The Chicago Community Trust to support enhanced patient care services between our clinic and local hospitals. This “Transitions of Care” initiative is supported by several local foundations and organizations to help LCHC collaborate with area hospitals. The goal of this initiative is to improve the health of low-income and uninsured patients through an innovative health data sharing system to address gaps in care, reduce duplication of services, and expand access to primary care services.
The partnership between Chicago Community Trust and LCHC will ensure our patients have access to an integrated healthcare experience and may reduce healthcare spending as a result. Lawndale Christian Health Center partners with local hospitals on Chicago’s west side. With this grant, Lawndale Christian Health Center will be able to improve collaborations with these key hospital partners, and more importantly, improve care for our patients.
