Apply for Medicaid

Illinois Expanded the Medicaid Program

In January 2014, Illinois made the Medical Card available to more individuals and families. The health care law provides Illinois with additional funding to expand its Medicaid Program to cover adults under 65 who make up to 138% of the federal poverty level. This is known as Medicaid Expansion.

Do you qualify?

Your coverage options depend on your income, household size, and other factors.  See the income limits for the Medical Card for different family sizes here.
Note: In most situations, pregnant women and children qualify for coverage in Illinois.

What you need to apply

  • A valid State ID or Driver's License
  • 1 month's worth of paystubs from all income sources
  • Social Security Number or proof that you applied for one

If you are a lawfully residing immigrant you will additionally need:

Proof of Citizenship

  • Permanent Resident Card or
  • Certificate of Naturalization/Citizenship

Proof of Illinois Residency

  • State ID or Driver's License or
  • Utility Bill (i.e. gas bill, water bill, electric bill)

How to Apply

Already Applied For Medicaid

Approval Process

Once your application is submitted it takes 30-60 days to hear a response from the State. After your application is approved for Medicaid, you will have the option to choose a health insurance plan through the Client Enrollment Broker Choice process. 

Client Enrollment Broker (CEB)

After you've been approved for Medicaid, the CEB will send an initial enrollment packet that will include a letter and "Your Health Plan Choices", a listing and description of every Health Plan offered within your area. There are three ways you can enroll in a health plan and choose a Primary Care Provider. 

  • By phone by calling the CEB at 1(877) 912-8880
  • By mail and filling out the paper enrollment form
  • Or online by visiting

CEB Enrollment Procedures

You have a 60 day enrollment period to pick a Health Plan and a PCP. If you do not select a Health Plan within the first 30 days of your enrollment period, Client Enrollment Services will mail you a second enrollment packet. This packet will identify and match you with a Health Plan along with a PCP. If you do not respond by the date given on the letter, the CEB will auto-assign a health plan and PCP for you. It is very  important that when filling out the paper enrollment forms or selecting a plan over the phone, to please remember the following list of insurance plans that we accept.

Did you already choose a health plan?

You can change your health plan anytime in the first 90 days. After that, you are "locked in" and you cannot change health plans for a year. If you are considering changing health plans please remember the following list of insurance plans that we accept

Open Enrollment 2019

The Health Insurance Marketplace, or more commonly known as "Obamacare" is a new way to find quality health coverage. You can compare plans based on price, benefits, and other features important to you side by side online! The Marketplace will also tell you if you qualify for free or low cost coverage. You can preview plans and estimate prices here.

You can generally buy Marketplace Health Insurance only during the annual Open Enrollment period. Upcoming dates to know: 

  • November 1, 2018: 2019 Open Enrollment Starts
  • December 15, 2018: 2019 Open Enrollment Ends

To buy Marketplace insurance outside Open Enrollment, you must qualify for a special enrollment due to a qualifying life event, experienced complex issues relating to the marketplace, or loss of other health coverage

Apply with a Special Enrollment

You can still buy private insurance through the Marketplace if you have a qualifying life event or a complex situation related to applying in the Marketplace. You generally have 60 days following life events that involved a change in family status or loss of other health care coverage to apply.

What is a Qualifying Life Event?

    • Moving from another state
    • Marriage
    • Divorce
    • Having a baby
    • Certain changes in income

What is a complex situation?

      • Enrollment error
      • Display errors on
      • Misconduct by an application assistor that resulted in not getting enrolled in a plan
      • Unexpected circumstances that kept you from enrolling
