Social Services

In addition to comprehensive medical care, LCHC also offers several social services to children and families. This includes helping families sign up for WIC, AllKids, free HIV Testing, free pregnancy testing, access to the New Birth Boutique and Case Management services.

Pregnancy & Newborn Case Management

You can choose to receive personal support from an LCHC pediatric Health Support Worker, who will assist you with both medical and non-medical needs during your pregnancy and through your child's important first year of life - this is called Case Management.  An infant’s first year of life is fundamental to his/her growth and development. During this important time in your child’s life, you can choose to receive personal support from an LCHC pediatric Health Support Worker, who will assist you with both medical and non-medical needs. Our trained staff provides additional education on such issues as immunization and breastfeeding, can deliver brief counseling, and help with developing effective coping skills. Our staff also coordinates needed services for medical insurance, WIC (nutrition), food and clothing, transportation, housing, daycare, and childhood development education. They can also help you by making appointments.

If you are interested in receiving these services, please call 872-588-3300.
